REFERRALS & APPOINTMENTS

APPOINTMENT SCHEDULING AND CANCELLATIONS:

Our patients can generally expect to receive an appointment within 24-48 hours of their request. We make every effort to meet urgent needs on the same day but this may require a waiting time of more than 15 minutes.

If patients must change or cancel their appointment, please call us 24 hours in advance so that we can extend this appointment time to another patient.

Initial appointments to our clinic require a physician's referral. Individuals, attorneys, occupational health nurses, case managers and insurance carriers can request that a physician make a referral to our clinic.

To initiate a referal to our clinic:

1. See your physician.

2. Ask physician to send us a referral.

3. Once we receive your referral we will contact you to schedule you initial appointment.

4. During your initial visit you will be evaluated and a care plan initiated.

5. You will be able to schedule future appointments at the end of your first visit.

INSURANCE:

Our team members will assist our patients in filing insurance claims. We participate in most managed care plans including Medicare. Payment arrangements can be made if there is no insurance coverage or limited coverage.
