PHYSICIAN REFERRAL:

Initial appointments* to our clinic require a physician's referral. Individuals, attorneys, occupational health nurses, case managers and insurance carriers can request that a physician make a referral to our clinic.

To initiate a referal to our clinic:

1. See your physician.

2. Ask your physician to send us a referral.

3. Once we receive your referral we will contact you to schedule your initial appointment.

4. During your initial visit you will be evaluated and a care plan initiated.

5. You will be able to schedule future appointments at the end of your first visit.

*If you are a patient and need to change or cancel an appointment, please call us 24 hours in advance so that we can extend this appointment time to another patient.
